Request for Healthcare Records
Clients have the right to request access to their healthcare records.
To request copies of your health care records, please submit a written request to Deborah Kirkland Counseling:

Include your full name, date of birth, description of the records requested, and preferred delivery method. I will respond within 15 business days of receipt of the request for records.
